Consent To Collect, Use, and Disclose Stories, Photos and/or Video and Sound Recordings
• I authorize The Lending Cupboard Society of Alberta (TLC) to record me and/or take my photo and use them in communications about TLC programs and services. I understand there are many ways of sharing communication, including printed and electronic methods. I understand that the recording or photo may be shared with a range of people and groups.
• I authorize TLC to use my name, address, and telephone number to contact me about this consent.
• I understand why these recordings and/or photos are being taken and how they may be used. I know that there are risks and benefits to giving this consent. I know that I can stop this consent at any time by informing TLC in writing.
• I understand that TLC cannot control information once it has been shared outside of TLC. I understand that asking TLC to stop using my recordings and/or photos will only stop the additional use of those recordings and/or photos after the date TLC receives my request.
• I agree to release and discharge TLC and those that TLC is responsible for at law, including the responsibility and liability for the content and claims for the printed/electronic communication where my information was used. I confirm that this release and discharge shall be binding upon my heirs, executors, administrators, and assigns.

Confidentiality Agreement

I have read and understood the Lending Cupboard Confidentiality Policy.

I understand that I may come in contact with confidential information during my time at The Lending Cupboard. As part of the condition of my work with the Lending Cupboard, I hereby undertake to keep in strict confidence any information regarding any client, employee or business of the Lending Cupboard or any other organization that comes to my attention while at The Lending Cupboard. I will do this per the Lending Cupboard’s privacy policy and applicable laws, including those requiring mandatory reporting.


I also agree never to remove any confidential material of any kind from the premises of The Lending Cupboard unless authorized as part of my duties or with the express permission or direction to do so from The Lending Cupboard.
